A problem solving analysis technique that seeks to find the root cause of trouble quickly. When a
self-evident problem arises, a question of why it happened is posed. Once the why question is answered, another why question is posed to address the previous answer. This continues until at least 5 levels of cause and effect for the problem are exposed. This generally provides an adequate overview of what is happening and points to a solution.
